Documents to be presented for settlement of claims:
1. A copy of passport and visa page of the Insured
2. Original receipt
3. Medical record, detailed expenditure sheet and a copy of examination report and laboratory test report of each respective treatment
4. Mailing Fee
5. Please go to the bank and ask the staff to print “Bank card customer information sheet(银行卡客户信息表) ” including the bank card number, customer name, branch name of the bank where you open the card.
